What to bring to your appointment
✔ Insurance card and photo identification to all appointments. ✔ List of your medications or the medications themselves ✔ Lateral neck x-ray, CT, or MRI, please try to bring the actual “films” or a CD itself ✔ Please bring copies of any other tests, such as recent blood work with you
